As a follow up to the guidance that L & I may recommend that certain jobs/roles  within our schools will need certain PPE, please complete the following questionnaire.

Your response will assist in determining who, when and how KN-95/N-95 masks/respirators need to be distributed in the building and in accordance with  L&I.
